粘膜麻醉在经食管心房调搏术中的应用 被引量:5

Application of mucomembranous anesthetization to transesophageal atrial pacing

摘要 采用1%Dicainae行食管电极周围粘膜麻醉,对20例在食管调搏术中有不同程度痛苦者进行用药后观察。用药后患者疼痛感减轻或消失。起搏阈值由25.2±7V降至19.4±6.4V(P<0.001)。而对心率、窦房传导时间、窦房结恢复时间、校正窦房结恢复时间、房室结功能不应期及有效不应期、左房有效不应期及相对不应期等电生理参数无明显影响(P>0.05)。笔者认为食管调搏术中应用粘膜麻醉可减少电刺激所致不良反应,有利于电极与食管壁接触,故而能降低起搏阈值,减轻病人痛苦。 In 20 patients with discomfortable feeling during transesophageal atrial pacing, we used 1% dicainae for mucomembranous anesthetization around pacing catheter electrodes, and then observed the effect on eletrophysiological measurements. It was found that the mucomembranous anesthetization with 1% dicainae obviously decreased pacing threshold from 26. 2±7 to 19. 4±6. 4 voltage (P<0. 001) and caused patients free of discomfortable feeling. There were no differences before and after mucomembranous anes thetization in heart rate, sinus node conduction time, sinus node recovery time, functional refractory period of atrioventricular node (AVN), effective refractory period of AVN, relative and effective refractory period of left atrium. It was concluded that mucomembranous anesthetization with 1% dicainae was good for patients with transesophageal atrial pacing by decreasing pacing threshold.
