超声引导下射频消融联合放射性粒子植入术治疗肝癌的临床研究

Clinical study on radiofrequency ablation combined with radioactive particle implantation under ultrasonic guidance in treatment of patients with hepatic cancer

目的探讨超声引导下射频消融(RFA)联合放射性粒子植入术治疗肝癌的临床价值。方法对2008年1月至2010年1月间在我院接受RFA联合放射性粒子植入术的肝癌患者30例进行前瞻性研究。结果患者均在超声引导下穿刺成功,内径小于3.0cm的结节均一次治疗完成,大于5.0cm的结节则分次治疗。治疗后患者腹胀、腹痛、纳差等症状明显减轻,自觉症状和心理状况明显改善;术后一个月肿瘤标记物检测甲胎蛋白(AFP)明显下降。所有病例均随访一年以上,无病例死亡。术后1周复查二维超声显示治疗前的低回声包块,治疗后可显示为等回声或者不均质的强回声。结论超声引导下射频消融联合放射性粒子植入术治疗肝癌的临床价值较大,值得应用。

Objective To study the clinical significance of radiofrequency ablation （RFA） combined with radioactive particle implantation under ultrasonic guidance in treatment of patients with hepatic cancer. Methods Sixty patients with cancer of liver received RFA combined with radioactive particle implantation in this hospital from January, 2008 to January, 2010 were prospectively studied. Results The stabs were all suc- ceeded under ultrasonic guidance. The treatment completed by one time in case of tubercle less than 3 centimeters in diameter and by many times in case of tubercle bigger than 3 centimeters in diameter. Symptoms of abdominal distension, stomachache etc in patients were alleviated, and gen- eral symptoms and psychologic status were obviously improved. The serum level of tumor marker AFP was obviously decreased in one month after operation. There was no death occurred during following - up period for one year. The two - dimension echography showed that low level echo of lump before treatment changed to equal echo or non - homogeneous dense echo after treatment. Conclusion The clinical significance of radiofre- quency ablation combined with radioactive particle implantation under ultrasonic guidance in treatment of patients with hepatic cancer is obvious and it is worthy to be widely applied.
