社区卫生服务机构老年人健康分级管理现况调查

Survey on Status of Graded Health Management for Elders in Community Health Service Institution

目的了解社区卫生服务机构开展老年人健康分级管理工作的现况。方法采用分层随机抽样的方法,抽取了成都市11家社区卫生服务中心,对其老年人健康分级管理的情况进行了调查;对11名机构负责人进行了个人深入访谈,了解其对老年人健康分级管理的认知和看法。结果有10家社区卫生服务中心开展了老年人健康分级管理,6家采用年龄结合健康状况的分级标准,3家采用以年龄为主的分级标准,1家采用以健康情况为主的分级标准;9家将老年人分为3级,1家分为4级。所有社区卫生服务中心统一使用社区卫生工作办公软件中的评估量表作为评估工作,均根据不同分级提供相应的健康服务。机构负责人认为老年人健康分级管理完善了对老年人的健康服务,提高了老年人的健康状况,提高了社区卫生服务中心的服务水平和知晓度;同时也认为在分级标准、人力资源、技术、资金、老年人的接受程度等方面还存在问题。结论老年人健康分级管理已在社区卫生服务机构不同程度地开展起来,在技术支持、资金支持、服务网络、服务对象等方面存在障碍。建议制订更有效的分级标准,提高工作人员的技术水平,拓展资金投入渠道,加大宣传力度,使社区老年人健康分级管理工作真正服务于广大老年人,切实提高其生活质量和健康水平。

Objective To learn about the situation of graded health management for the elderly persons in community health service institution.Methods The stratified sampling method was taken to select 11 community health services （CHS） centers in Chengdu.The situation of graded health management for the elderly was surveyed.The personal interview was conducted among the directors of the 11 CHS centers about their recognition and views of graded health management for the elderly.Results There were 10 CHS centers where the graded health management for the elderly had been carried out.Of which there 6 centers where the grades were classified according to age combining with health status,3 centers where the grades were classified mainly according to age;9 centers where the elderly persons were classified into three grades,and 1 center,into four grades.The assessing work was conducted by the unified assessment scale of CHS office software in all the CHS centers,and the corresponding health services were given according to the different grades.The directors held that the graded management improved the health services for the elderly and raised the health conditions for them,thus increased the service level and awareness of CHS centers.Nevertheless the directors also believed there still were some problems in grade criteria,human resource,skill,fund and acceptance by the elderly.Conclusion The graded health management for the elderly has already been carried out in CHS institutions to some extent.However,there are some barriers in technique support,found,and service network and service object.It is necessary to formulate the more effective grade criteria,improve the staff＇s＇ skills,broaden financing channels,and enhance propaganda to push forward the graded health management for the elderly,thus really raising the quality of life and health level for the elderly.
