恒牙初期安氏Ⅱ~1分类高角型错的X线头影测量分析

Cephlometric analysis of class Ⅱ~1 malocclusion of high-angle group in adolescences

【目的】通过对安氏Ⅱ1分类错高角骨面型组与正常组X线头影测量的分析,观察比较安氏Ⅱ1分类高角型错的颅面特征,探讨安氏Ⅱ1分类错高角骨面型的颅面特征及在正畸治疗中控制侧重点,为安氏Ⅱ1分类错高角骨面型的临床诊断与矫治设计提供理论依据。【方法】本研究选择恒牙初期正常60例和安氏Ⅱ1分类高角错42例的头颅定位侧位片,分别对颅面41个测量项目进行X线头影测量,并对各测量项目的均值进行统计学比较和分析。【结果】安氏Ⅱ1分类错高角组颅底的曲度增大,颅前基底向前上旋转;上、下颌骨较颅底的矢状向关系均显后缩,且下颌后缩更为明显;上颌后牙槽高度U6-PP(mm)减小;下颌后牙槽高度L6-PP(mm)增加;上前牙唇倾,下前牙直立,上下切牙间角与正常组存在显著性差异。【结论】恒牙初期安氏Ⅱ1分类高角型错患者颅面结构具有显著特点,临床中应根据其特点做出正确的诊断设计并选择合理的治疗方法。

[Objective]To observe and compare the craniofacial characteristics of Ⅱ1 malocclusion of high-angle group and the type of orthodontic treatment in the control of focus,for the Ⅱ1 malocclusion of high-angle group of the surface of clinical diagnosis and treatment reference design theory.[Methods]A total of 60 normal occlusion,42 Ⅱ1 malocclusion of high-angle group were chose.All of them are in early permanent teeth period.We make cephalometry of the lateral projection by computer and analysis the measured value between the normal occlusion and the Angle class Ⅱ1 malocclusion of high-angle group and face type.[Results]In Ⅱ1 malocclusion of high-angle group,the angle of N-S-Ar and the basic ranial curvature increased;maxillary and mandible both retraction,especially in mandible;the height of upper molar were decreased,but the lower molar increased;the up-down incisor teeth angle increased significantly.[Conclusions]In early permanent teeth period,there are significant face types of angle class Ⅱ1 malocclusion of high-angle group.So we must make correct diagnosis and reasonable treatment on the basis of the characteristic of high-angle group in the orthodontic clinic.
