DHS和PFNA治疗老年骨质疏松患者股骨粗隆间骨折疗效分析 被引量:13

The Clinical Effect of DHS and PFNA on Intertrochanteric Fracture of Femur in Elderly Osteoporosis Patients

摘要 目的:比较治疗股骨粗隆间骨折的2种常用方法的治疗效果。方法:选择住院部收治的患者147例,分别应用DHS及抗旋股骨近端髓内钉(PFNA)手术方法进行治疗,观察手术切口长度、手术时间、术中平均出血量、平均愈合时间及Harris髋关节功能评分等指标。结果:2组患者在手术内固定的过程中均没有并发症发生,术后拍摄X线片复查,骨折复位良好,固定牢固,随访3~36个月,147例骨折全部愈合,与PFNA手术方法比较,DHS平均手术时间、术中平均出血量明显增加、Hariris髋关节功能评分比较差异有统计学意义(P<0.05)。平均骨折愈合时间比较差异无统计学意义(P>0.05)。结论:相对于DHS,PFNA治疗股骨粗隆间骨折具有操作简单,出血少,手术时间短,固定可靠,尤其适用于骨质疏松老年患者的股骨粗隆间骨折。 Objective:To study the clinical effect of DHS and PFNA on intertrochanteric fractures in elderly osteoporosis patients. Methods:A total of 147 elderly osteoporosis patients with intertrochanteric fractures were treated randomly with PFNA (82 patients) or DHS internal fixation (65 patients). Main outcome measurements included the incision length,the operation time,blood loss,average fracture healing time,and functional outcome (Harris hip score),etc. Results:No complications occurred in either group. Compared with DHS group,better results were observed in PFNA group about the operation time,blood loss and Harris hip scores,and the differences were statistically significant (P〈0.05). Conclusion:PFNA can be recommended to treat intertrochanteric fractures in the elderly osteoporosis patients because of satisfactory effect and high quality of life after the operation.
