护理告知及知情同意书对防范血透中心护患纠纷的作用

Effect of informing and informed consent system on prevention of medical disputes in hemodialysis center

目的落实血透中心患者护理告知、知情同意书制度,降低护理纠纷及投诉发生率。方法按时间顺序将590例血透住院患者分为观察组(300例)和对照组(290例)。对照组按常规方式宣教;观察组采用口头护理告知,并自行设计包括5个系列28个条目的告知与知情同意项目书。采取书面告知、图文宣传告知及多媒体讲座告知等形式,对患者进行宣教。结果两组除环境设施与制度所致的护患纠纷外,因安全防护措施、护理操作、治疗不良反应及相关费用所致的护患纠纷发生率比较,差异有统计学意义(P<0.05,P<0.01)。结论履行护理告知义务、尊重患者知情同意权可减少护患冲突和护患纠纷。

Objective To reduce the occurrence of medical disputes in hemodialysis center by nurses＇ implementing informing and informed consent policy. Methods Totally, 590 hemodialysis patients were divided into an observation group （n 300） and a control group （n=290） in chronological order. The control group were given conventional health education, while the observation group were subjected to a variety of education by using oral approach, official papers, pictures, and multi-media lectures based on a selfdesigned informing and informed consent protocol which included 5 series, totaling 28 items. Results Except for the aspect of envi ronment and policy, there were significant differences in rates of medical disputes in association with safety precaution, nursing service, adverse reaction of treatment, and medical expense between the two groups （P〈0.05, P〈0.01）. Conclusion Performing the obligation of informing and respecting patients＇ right of informed consent can reduce the nursing-related disputes.
