有限内固定结合外固定架治疗C3型Pilon骨折

Treatment of C3 Type Pilon's Fractures with External Fixation combined with Limited Internal Fixation

目的探讨有限内固定结合外固定架治疗C3型Pilon骨折的疗效。方法对22例C3型Pilon骨折分别采用有限内固定结合外支架固定方法进行治疗。14例予以骨植骨修补骨缺损。其中大块髂骨植入7例,自体松质骨填塞7例。结果所有病例均获得随访,随访时间12～30个月,平均20个月。参照Mazur制订的踝关节功能评分标准,优14例,良6例,可2例。结论有限内固定结合外固定架是治疗C3型Pilon骨折的一种有效方法,同时大块髂骨植骨能有效解决严重骨缺损,有利于功能恢复。

Objective To analyze the effect of limited internal fixation combined external fixation for Pilon fractures with C3 type.Methods Twenty-two patients with C3 type Pilon′s fracture were treated by external fixation combined with limited internal fixation.Fourteen cases were treated by bone transplantation（big pieces of ilium was 7,spongy bone was 7）.Results All cases were followed up for twelve to thirty months according to the ankle score of Mazur,the results were excellent in 14 cases,good in 6,fair in 2.Conclusion Limited internal fixation combined with external fixation has some advantages of fewer complications and function recovery in the treatment of Pilon fracture of C3 type,and the bone transplantation with big pieces of ilium is effective to solve the serious bone loss,and exercise ankle joint earlier.
