灯盏花素联合氯沙坦治疗早期糖尿病肾病的临床观察

Clinical Observation on Therapeutic Effects of Combined Use of Dengzhanhuasu and Losartan in Treating Early Diabetic Nephropathy

目的:观察灯盏花素与氯沙坦联合治疗早期糖尿病肾病的疗效。方法:将80例糖尿病肾病患者随机分为治疗组42例和对照组38例,两组均予常规治疗,氯沙坦口服每次100mg,每日1次,治疗组另予灯盏花素40mg,加入生理盐水250mL,静脉滴注共两个疗程。观察两组临床症状,尿微量白蛋白排泄率(UAER),尿B2-微球蛋白(B2-MG),纤维蛋白原(FIB),D-二聚体(D-D)指标变化。结果:治疗组总有效率76.2%,对照组总有效率44.2%,两组比较有统计学差异(P<0.05)。治疗后两组UAER,尿B2-MG均有所下降,与治疗前比较差异有统计学意义(P<0.05);且治疗组与对照组比较,差异有统计学意义(P<0.05);治疗后治疗组FIB,D-D显著下降,与治疗前及对照组比较,差异有统计学意义(P<0.05),对照组治疗前后差异无统计学意义(P>0.05)。结论:灯盏花素与氯沙坦联合治疗早期糖尿病肾病有明显疗效。

Objective： To observe the clinical effects of combined use of dengzhanhuasu and losartan in treating early diabetic nephropathy（DN）. Methods： Eighty patients with DN were randomly assigned to the theatment group 42cases and the control group 38 cases. Conventional treatment was given to both groups, but to the control group 100 mg losartan was given orally once a day additionally, while to the treatment group the same dosage of losartan po. and 40mg/d of dengzhanhuasu injection by adding in 250mL 0.9% normal saline for intravenous dripping. Changes of clinical symptoms, urinary albumin excretion rate （UAER）, urinary levels of β2-microglobulin（ β2-MG）, D-dimer（D-D） and plasma fibrinogen （FIB） were observed. Results： The total effective rate in the treatment group was higher than that in the control group （76.2% vs 44.2%, P〈0.05）. Levels of UAER, β2-MG in urine were decreased significantly after treatment in both groups, showing significant difference as compared with those in the control group （P〈0.05）; while decrease of D-D, FIB only happened in the treatment group （P〈0.05）, so the post-treatment data in the treatment group were significantly lower as compared with those in the control group （P〈0.05）.ConcIusion： The Combined Use of Dengzhanhuasu and Losartan in treating early diabetic nephropathy is effective.
