奥沙利铂过敏49例分析

Analysis of hypersensitivity reactions to oxaliplatin in 49 patients

目的探讨奥沙利铂过敏的临床表现和应对策略,为临床安全用药提供参考。方法回顾性分析本院2003年6月—2009年5月奥沙利铂过敏病例共49例,对其初次出现过敏的时间、症状以及对原治疗计划的影响进行比较分析。结果奥沙利铂过敏反应发生率为1.7%,平均累计剂量为(540±s157)mg·m-2。35%过敏患者应用奥沙利铂30min内出现症状,其余患者用药1～24h出现症状。过敏患者中Ⅳ度过敏症状发生率为2%。结论应用奥沙利铂24h内应密切关注其不良反应发生的可能性,Ⅰ～Ⅲ度过敏患者再次用药前30min应给予预防性抗过敏药物并减慢静脉滴注速度,一般无需中断治疗。

AIM To explore the clinical manifestation and solution oxaliplatin in order to provide a reactions to oxaliplatin observed guide to the safe use of the drug. in our hospital from June 2003 to METHODS of hypersensitivity reactions to Patients （n = 49） with allergic May 2009 were analyzed retrospectively. The first onset time, manifestation and impact on treatment planning were compared. RESULTS The incidence of hypersensitivity reactions to oxaliplatin was 1.7%, and the average cumulative dose was （540 ± s 157） mg.m-2. Thirty-five percent of allergic reactions occurred within 30 rain, and the others occurred within 1 - 24 h after the administration. Two percent of patients experienced a grade Ⅳ hypersensitivity reaction. CONCLUSION Close attention must be paid to the patients who received oxaliplatin within 24 h. Oxaliplatin should be administered again 30 rain after the use of antiallergic drugs and infused slowly in patients who experienced a grade Ⅰ -Ⅲ hypersensitivity reaction, and need not to be stopped.
