上消化道异位胰腺的临床特征分析

Clinical features of upper gastrointestinal heterotopic pancreas

目的总结上消化道异位胰腺的临床特征。方法对23例上消化道异位胰腺的一般资料、临床表现、胃镜下表现、EUS特征及病理学改变进行分析。结果23例上消化道异位胰腺常见的临床症状为上腹痛9例(39.1%),腹胀7例(30.4%),嗳气4例(17.4%),反酸、烧心3例(13.0%)。异位胰腺位于胃窦19例,胃体前壁2例,十二指肠球部2例。EUS上消化道异位胰腺声像学主要特征为:病变多起自黏膜下层,内部回声不均匀,可见不规则的中等回声,边界清楚无包膜。高频电凝切除术后未出现出血、穿孔等并发症。结论EUS对上消化道异位胰腺的诊断具有重要价值,内镜下切除是一种安全、有效的治疗手段。

Objective To summarize clinical features of upper gastrointestinal heterotopic pancreas.Methods General date,clinical manifestation,endoscopic manifestation,EUS characters and pathological findings were analyzed in 23 cases of upper gastrointestinal heterotopic pancreas.Results The common clinical symptoms of 23 cases of upper gastrointestinal heterotopic pancreas were upper abdominal pain（9 cases）,bloating（7 cases）,belching（4 cases） and acid reflux heartburn（3 cases）.Heterotopic pancreas located in sinuses ventriculi 19 cases,the anterior wall of gastric body 2 cases,duodenal bulb 2 cases.The key feature of EUS acoustic images of upper gastrointestinal heterotopic pancreas was that most of lesions were from submucosa and their internal echo was asymmetric and irregular medium echo.No complications such as hemorrhage and punch were found after high-frequency electric excision.Conclusion EUS is valuable in the diagnosis of upper gastrointestinal heterotopic pancreas and the treatment with endoscope is safe and effective.
