双极人工股骨头置换治疗高龄患者围手术期护理

Nursing care of aged gerontal femoral neck fracture patients after artificial femoral head replacement

目的：总结人工股骨头置换术治疗高龄患者的股骨颈骨折的围手术期护理效果。方法：对采用双极人工股骨头置换术治疗53例老年股骨颈骨折患者，术前对患者进行全身情况评估、有针对性地严密观察病情，订出个体化的积极预防并发症措施，术后早期进行功能康复锻炼。结果：随访6月～36月，按Harris髋关节评分标准：优40例，良11例，中2例，优良率96．23％（51／53）；术后髋痛率22．64％（12／53）。53例均在术后1～3周恢复下地行走，无一例伤口感染及围手术期死亡；患者均获得生活自理。结论：人工股骨头置换术是高龄患者股骨颈骨折的有效治疗方法，只有加强围手术期的治疗和护理，才能减少高龄股骨颈骨折患者并发症及死亡率，并提高其生活质量。

Objective To summarize the nursing care effect of aged gerontal femoral neck fracture patients after undergoing artificial femoral head replacement. MethodsThe nursing care effect of 53 gerontal femoral neck fracture patients was investigated after undergoing artificial femoral head replacement by evaluating perioperative situation, watching the pathogenetic condition, reducing complications, and conducting early rehabilitation exercise. Results Followed up for 6 to 36 months,according to the Harris standardization the results were 40 cases excellent, 11 good and 2 middle,with a total of 96.23%（51/53） excellent or good. 22.64%（12/53） of them feel pain in the hip. All of them can walk in 1 to 3 weeks after the operation,self--care was achieved,and none die. Conclusion Artificial femoral head replacement was effective for gerontal femoral neck fracture patients. Their quality of life was raised because of the perioperative nursing care.
