诺杨在术后镇痛中的不同用法探讨 被引量：9

摘要 目的:比较诺杨(Butophanol)在术后镇痛中几种常用方法的临床效果和不良反应,为其在临床上的应用提供一定的参考。方法:100例ASAⅠ、Ⅱ级下腹部和下肢手术全麻患者随机分为4组,每组20例,采用PCIA(Ⅰ、Ⅱ、Ⅲ组)和PCEA(Ⅳ、Ⅴ组)两种镇痛方式,镇痛配方为:Ⅰ组Butophanol 10mg+Tropisetron(托烷司琼)10mg;Ⅱ组Butophanol 5mg+Tropisetron 10mg;Ⅲ组Tropisetron10mg;Ⅳ组0.75% Naropine(罗哌卡因)150mg+Tropisetron 10mg;Ⅴ组Butophanol 5mg+0.75% Naropine 150mg+Tropisetron 10mg。记录术后6、12、24和48h四组患者Ramesay(镇静)、VAS(镇痛)评分及Ⅳ、Ⅴ组Bromage(下肢运动)评分并进行比较,记录不良反应的发生情况。结果:同Ⅲ组相比,Ⅰ、Ⅱ组患者在各时段的VAS评分较低,Ramesay评分较高,差异有显著性(P<0.05),同Ⅱ组相比,Ⅰ组患者的VAS评分较低,Ramesay评分较高,差异有显著性(P<0.05),同Ⅰ组相比,Ⅴ组患者的VAS评分差异无显著性(P>0.05),Ramesay评分则较低(P<0.05),Ⅳ和Ⅴ组的VAS评分差异有显著性,后者较低(P<0.05),Ramesay评分和Bromage评分差异无显著性(P>0.05)。结论:诺杨可安全、有效地用于术后镇痛,但镇静作用较强,需加强临床监测。
