孕妇血清IgG血型抗体与新生儿溶血病关系探讨

Investigation on the relation between antibodies of gravida serum IgG blood type and hemolytic disease of newborn

对１１５４例孕妇及丈夫作ＡＢＯ、Ｒｈ（Ｄ）血型检查。有ＡＢＯ血型不合者，测ＩｇＧ抗Ａ（Ｂ）抗体效价；Ｒｈ（Ｄ）阴性者，查ＩｇＧ抗Ｄ抗体。又对其中２０４例产前有新生儿溶血病（ＨＤＮ）危险的孕妇（即产前ＡＢＯ不合，ＩｇＧ抗Ａ（Ｂ）≥６４或有Ｒｈ抗体者），所生新生儿作血型及３项试验，以证实产前ＩｇＧ抗体效价高低与ＨＤＮ发病率的关系。结果：ＡＢＯ系不合者占４２％，妇夫血型不合检出率依次为ＯＡ＞ＯＢ＞ＯＡＢ，效价≤６４占７５５％，≥５１２占１２％；Ｒｈ阴性检出率为１３８％，阴性产生抗Ｄ机率为５２５％，２０４例产前预报有ＨＤＮ可能的孕妇中，有３８例新生儿患有ＨＤＮ，占１８６％。

The ABO,Rh(D) blood types were tested in 1 154 gravidas and their husbands,and IgG anti A(B) valence of antibody and IgG anti D antibodies were measured when ABO blood type was disagreement and Rh(D) negative,respectively.The blood type and three experiments were done in 204 gravidas who had risk of hemolytic disease of newborn (HDN) (ie.ABO disagreement,IgG anti A(B)≥64 or positive Rh antibodies),to determine the relation of valence of IgG antibody and disease incidence morbidity of HDN.The results showed that the disagreement rate in ABO series was 42%,in proper detection rates in wife and husband O A>O B>O AB order,valence(≤64) 75 5% and (≥512) 1.2%.Detection rate of negative Rh was 1.38%,and negative producing anti D probability 52.5%,and HDN of neonate(38 cases)18.6% in predictin possibility of HDN of 204 gravidas before birth.It is suggested that the magnitude of valence of IgG blood type antibody is directly proportional to incidence of HDN.
