老年人胃癌的特点及外科治疗

The characteristics of the elderly and surgical treatment of gastric cancer

目的探讨老年人胃癌的特点及外科手术治疗方法。方法分析我院1998年10月至2008年10月收治并经手术及病理检查证实的70岁以上胃癌患者85例的临床资料。结果本组38例术后发生各种并发症56例次。包括肺部感染、多脏器功能衰竭、吻合口瘘、切口裂开、切口感染、尿路感染等。围手术期死亡5例,死亡原因为呼吸功能衰竭、多脏器功能衰竭、吻合口瘘并发腹腔感染、水电解质紊乱。本组病例除5例围手术期死亡外,其余均获随访,随访时间1～6年,平均2.3年。本组平均存活期为11.3个月,最长1例已存活6年。结论加强围手术期处理,尽可能实施手术治疗是提高老年胃癌患者生存率的关键。

Objective To explore the surgical treatment of gastric cancer aged characteristics.Methods Analysis of our hospital from October 1998 to October 2008,and were treated by surgery and pathological examination confirmed gastric cancer patients over 70 years old clinical data of 85 cases.Results 38 cases of postoperative complications of 56 cases of times.Including pulmonary infection,multiple organ failure,anastomotic leakage,wound dehiscence,wound infection,urinary tract infections.Perioperative mortality in 5 cases,cause of death was respiratory failure,multiple organ failure,anastomotic fistula in intra-abdominal infections,water and electrolyte disturbance.Espect these five cases,the rest have all been followed up for 1~6 years,an average of 2.3 years.The average survival period of 11.3 months,the longest one cases has survived 6 years.Conclusion Strengthening the perioperative management,as far as surgical treatment is the key to improve the survival rate of elderly patients with gastric cancer.
