低血糖致急性脑功能障碍28例急诊诊治分析

Analyzing the diagnosis and treatment of 28 cases with acute brain dysfunction resulted from hypoglycemia

目的探讨低血糖致急性脑功能障碍的急诊诊断和治疗。方法回顾性分析本院急诊科28例低血糖致急性脑功能障碍患者的临床资料。结果本组28例中,男18例,女10例。服用降糖药者25例,无糖尿病史者3例。伴脑梗死病史20例,消化性溃疡史2例,胆囊炎史者1例,经诊治后24例痊愈,3例后遗脑功能障碍,1例死于多器官功能衰竭。结论糖尿病患者出现意识障碍伴大汗,首先要考虑低血糖的可能。患有慢性疾病的患者要避免诱发低血糖的相关因素。

Objective To study the diagnosis and treatment of acute brain dysfunction resulted from hypoglycemia.Methods The clinical data of 28 patients with acute brain dysfunction resulted from hypoglycemia were analyzed retrospectively in our Emergency Department.Results 28 cases in total,18 cases were male and 10 cases were female.25 cases were patients with diabetes mellitus who were receiving drug treatment;3 cases were patients without diabetes mellitus.20 cases suffered from cerebral infarction;2 cases suffered from peptic ulcer and 1 case suffered from cholecystitis.24 cases recovered completely after treatment while 3 cases got brain dysfunction and 1 case died of multiple organ dysfunction syndrome（MODS）.Conclusion When patients with diabetes mellitus appear conscious disturbance and hyperhidrosis,a diagnosis of hypoglycemia should be considered first.The risk factor that can induce hypoglycemia should be avoided.
