隐匿性肾小球肾炎62例临床病理分析及预后

目的探讨隐匿性肾小球肾炎临床与病理的关系,进行危险因素及相关性分析,提高对该病的认识水平。方法对62例临床诊断为隐匿性肾炎的患者均行肾活检穿刺术及实验室检查,肾组织行光镜、免疫组化及电镜检查。结果单纯性血尿14例（22.6%）;单纯性蛋白尿18例（29.5%）,蛋白尿伴血尿30例（48.4%）;IgA肾病37例（59.7%）,增生性肾病14例（22.6%）,轻微性肾病5例（8.1%）,局灶节段性肾病3例（4.8%）,膜性肾病2例（3.2%）。合计轻度病变47例（75.8%）,偏重病变15例（24.2%）。蛋白尿伴血尿组肾病的发生率最高（P〈0.05～0.01）,其病理改变也相对较重;结论隐匿性肾炎病理类型多样,但以IgAN为主,蛋白尿伴持续镜下血尿与病情严重程度明显相关。

Objective To investigate the relationship between clinical and pathological of latent glomerulonephritis, improve the awareness level of the disease. Methods 62 cases diagnosed latent glomerulonephritis patients underwent renal biopsy puncture and routine examination, kidney tissue was checked with microscopy, immunohistochemistry and electron microscopy. Results There were 14 cases with single-hematuria （22.6%）; 18 cases （29.5%） with single-proteinuria, and 30 cases （48.4%） with hematuria-proteinuria; There were 37 cases of IgAN（59. 7%）. Totally, there were 47 cases of mild type （75.8%） and 15 cases of severe type （24.2%）. The group of proteinuria associated with hematuria exhibited the highest incidence of nephropathy （P〈 0.05 ~0.01）, the pathological changes were also relatively heavy. Conclusions Although the pathological types were varied, main type was IgAN. Proteinuria associated with henaturia was significant correlated with glomerulonephritis.
