摘要
目的观察小肠脂酸结合蛋白基因(FABP2)密码子54变异与体脂含量及分布的关系。方法165例中国人,糖耐量正常(NGT)者86人,2型糖尿病患者(DM)79人。用PCR/HhaI酶解检测FABP2密码子54变异的等位基因———苏氨酸54(Thr54)及丙氨酸54(Ala54)。比较NGT及DM组间及组内体脂含量亚组(BMI)及体脂分布亚组(核磁共振局部体脂测定)间的FABP2密码子54频率分布。结果DM股部皮下脂肪(FA)亚组间FABP2密码子54变异基因型频率分布有显著差异(χ2=11.46,P=0.003)。表现为FA<75cm2者Thr54(+)基因型频率显著增高(χ2=10.11,P=0.001;比数比4.62),及Thr54等位基因型频率显著增高(χ2=5.379,P=0.020)。FA相关因素的多元逐步回归分析中见到除性别(P=0.0001)、BMI(P=0.0001)及血总胆固醇(P=0.004)外,FABP2亦是独立相关因素(P=0.003)。FABP2相关因素的Logistic回归分析中,仅见FA与之相关(P=0.029)。结论FABP2与体脂分布相关。因为DM不仅有腹内脂肪增多,且有股部?
Objective To investigate the association between the codon 54 variation of intestinal fatty acid binding protein (FABP2) gene and the amount and the distribution of adipose depots. Methods In 165 Chinese subjects 86 with normal glucose tolerance (NGT), and 79 with type 2 diabetes (DM) . The alleles of the codon 54 variation of FABP2 gene allele threonine 54 (Thr54) and allele alanine54 (Ala54) were detected by using PCR/Hha I digestion. The frequencies of the FABP2 codon 54 variation were compared between NGT and DM groups, and also between body fat subgroups (expressed as BMI) and body fat distribution subgroups (expressed as regional adipose tissue area measured by magnetic resonance imaging) within NGT and DM groups. Results The genotype frequencies of FABP2 codon 54 variation were significantly differentbetweenfemoralsubcutaneousadiposetissue(FA)subgroupsinDMsubjects (χ 2=11.46,P=0.003). DM subjects with FA<75 cm 2 had higher frequency of Thr54 (+) genotype (χ 2=10.11,P=0.001; odd ratio=4.62) and higher frequency of Thr54 allele (χ 2=5.379,P=0.020; odd ratio=2.36). Stepwise regression analysis showed that in addition to sex (P=0.0001), BMI(P=0.0001) and serum total cholesterol (P=0.004), FABP2 codonvariationwasalsoanindependentfactoraccount for FA (P=0.003). Logistic regression analysis revealed that only FA was associated with FABP2 codon 54 variation (P=0.029). Conclusion FABP2 gene was correlated with body tissue adipose distribution. In DM, not only increased intra abdominal fat, but also decreased femoral fat were observed. This study showed that the decreased femoral fat depot in DM subjects was related to FABP2 Thr54 (+) genotype, which suggests that FAPB2 gene may take part in the pathophysiological process of diabetes mellitus.
