利用MRI探讨TMJ关节镜术后咬合恢复的影响因素

摘要 目的:利用MRI探讨经颞下颌关节镜下盘复位固定术后患者咬合关系自行恢复的可能影响因素。方法:按关节镜术后4周咬合关系能否自行恢复,将患者分为暂时错及长期错2组,每组包括30例单侧手术病例。所有患者术前均行MRI检查,读片记录2组患者MRI所显示关节盘形态及髁突骨质变化情况,应用SAS6.12软件包比较2组间的差异,探讨术后错恢复的可能影响因素。结果:2组患者的关节盘形态及髁突骨质变化情况存在显著性差异(P<0.05),长期错组患者与暂时错组相比,关节盘变形程度及髁突骨质变化情况均显严重。结论:通过术前MRI所显示关节盘形态及髁突骨质变化情况,可以初步预计患者术后咬合关系能否及时恢复,从而及时进行相应的临床干预。 PURPOSE:Use of MRI to evaluate the factors influencing the occlusal recovery after TMJ arthroscopic disc reposition and fixation.METHODS:Sixty patients were divided into 2 groups:temporary malocclusion group and lonetime malocclusion group,according to their malocclusion whether to recovered in 28 days after TMJ arthroscopic disc reposition and fixation.Each group included 30 cases who underwent unilateral TMJ arthroscopic disc reposition and fixation.MRI was used to assess the morphology of disc and osseous changes of the condyle.The data were compared statistically using SAS6.12 software package between two groups.RESULTS:The disc configuration,osseous change in condyle surface showed significant differences between 2 groups(P〈0.05).The patients in long-time malocclusion group had more severe deformation of disc and more serious condylar resorption than patients in temporary malocclusion group.CONCLUSION:It can be predicted whether patient's malocclusion will recover in time according to disc morphology and condylar bone changing displayed by MRI.
