大骨瓣减压救治重型高血压脑出血50例临床分析

目的探讨大骨瓣减压术治疗重型高血压脑出血患者的临床效果。方法回顾性分析我院神经外科对2002年1月～2009年6月间收治的50例重型高血压脑出血患者的临床资料,总结治疗经验。结果本组50例患者,术后存活40例,死亡10例,死亡率为20%;50例中血肿清除率80%以上41例(82%),低于80%9例(18%);根据日常生活能力(ADL)分级法Ⅰ级6例,Ⅱ级18例,Ⅲ级13例,Ⅳ级2例,Ⅴ级1例,恢复良好率为92.5%。结论超早期大骨瓣减压血肿清除术是一个行之有效的外科治疗方法。

Objective To investigate the clinical effect of large craniectomy in the treatment of the patients with severe hypertensive intracerebral hemorrhage.Methods Retrospective analysis the clinical data of 50 patients with severe hypertensive cerebral hemorrhage in our hospital neurological surgery between 2002.1～2009.6,summarizing the treatment experience.Results The group of 50 patients,postoperative survival of 40 cases and 10 died,the mortality rate was 20%;More than 80% clearance rate of 41 cases （82%）,less than 80% in 9 cases （18%） in the group;according to activities of daily living （ADL） classifi cation scheme 6 cases of gradeⅠ,Ⅱgrade 18 cases,Ⅲgrade 13 cases,Ⅳgrade 2 cases,Ⅴlevel 1 case,a good recovery rate was 92.5%.Conclusion The ultra-early large craniectomy hematoma is an effective surgical treatment.
