RPH与PPH治疗Ⅲ度脱垂痔临床分析

Clinical Analysis of Automatic Ligation of Superior Hemorrhoidal Mucosa and Procedure for Prolapse and Hemorrhoids in the Treatment of Ⅲ Degree Prolapsed Hemorrhoids

为比较Ruiyun式可吸入式套扎器痔上黏膜套扎术（RPH）与吻合器痔上黏膜环切术（PPH）治疗Ⅲ度脱垂痔的手术效果，将78例Ⅲ度脱垂痔分成R组（RPH）42例和P组（PPH）36例，分别采用RPH和PPH。术后观察7d，并随访1～3个月。比较两组平均手术时间、手术完成后痔核脱垂情况、创口愈合时间、住院时间等结果。结果显示，平均手术时间R组明显优于P组（P〈0．001）。术后肛门坠胀R组明显轻于P组。术后晚期出血R组多于P组。尿潴留、术后脱垂状态两组差异无统计学意义。结果表明，RPH治疗Ⅲ度脱垂痔具有手术时间短、并发症少等优点。

The objective of this study was to understand the efficacy of automatic ligation of superior hemorrhoidal mucosa by using Ruiyun-type ligator （Ruiyun procedure for hemorrhoids, RPH） and procedure for prolapse and hemorrhoids（PPH） in the treatment of Ⅲ degree prolapsed hemorrhoids. The 78 patients with Ⅲ degree prolapsed hemorrhoids were divided random into two groups, 42 cases of them were R （RPH） group and 36 cases were P （PPH） group,and they receive RPH and PPH respectively. They were observed for 7 days at postoperation, and received the follow-up of 1 month to 3 months at postoperation. Average time of operation,postoperative hemorrhoids prolapse state,time of wound healing,time of hospital stay,and so on were compared between both groups. As results,R group was superior to P group about the average operation time（ P 〈0. 001）. And there was significant difference about distend tenesmus of anus,R group was less than in the P group. But,about late postoperative bleeding,R group was more than P group. And urine retention,postoperative prolapse state was statistically no difference between them. Results show that RPH possess a short operation time and a fewer complications,etc.
