摘要
目的研究脓毒症大鼠急性肾功能衰竭时中性粒细胞表面CD18和血清TNF—α浓度的变化以及地塞米松(DXM)对其影响。方法72只雄性SD大鼠随机数字表法分成对照组(A组)、脂多糖(LPS)模型组(B组)、DXM+LPS治疗组(C组),各组在2、4h和6h时观察8只大鼠肾组织大体外观及病理学变化,测定血肌酐浓度的变化,中性粒细胞表面CD18的表达,以及用ELISA法检测血清TNF—α水平变化。结果B组肉眼观肾脏表面色泽暗红,可见包膜下点状、片状出血;C组肾脏表面有轻度充血、水肿;与A组比较,B组肾脏病理呈急性炎症改变,C组肾脏病变较B组明显减轻。在4、6h时点,B、C组血肌酐浓度与A组比较明显上升(P〈0.05),但在4、6h时点B组血肌酐浓度高于C组(P〈0.05)。B组中性粒细胞表面CD18表达量(平均荧光道数)和血清TNF—α浓度较A组明显升高(P〈0.05),C组较B组中性粒细胞表面CD18表达量和血清TNF—α浓度显著降低(P〈0.05)。结论DXM可以通过下调中性粒细胞表面CD18表达量和血清TNF—α浓度,抑制中性粒细胞在肾脏的浸润而保护脓毒症大鼠的肾功能。
Objective To investigate the effect of dexamethasone (DXM) on peripheral neutrophil CD18 and serum TNF- α in septic rats with renal failure. Methods Animal model of sepsis with acute renal failure was induced by intravenous injection of lipopolysaccharide (LPS) in male SD rats. Seventy-two male SD rats were randomly divided into three groups: group A (normal group, n=24),group B (LPS group, n=24) and group C (LPS+DXM,n=24). The pathological changes of kidney tissue, blood creatinine concentration, serum TNF- α were examined at 2h, 4h and 6h after LPS was given. Results The pathological changes of kidney tissue in group C were milder than those in group B at 6 hours after LPS given. Blood creatinine concentrations in group B and group C were higher than those in group A at 4 and 6 h after LPS given; while those in group C were lower than in group B at 4 and 6 h after LPS given. Serum TNF-α levels and neutrophil CD18 in group C declined more markedly than those in group B at 4 and 6 h after LPS given. Conclusion Dexamethasone can protect the renal function in LPS-induced septic rats, which is likely to be related to the downregulation of neutrophil CD18 and TNF- α.
