摘要
目的:讨论2型糖尿病(T2DM)患者血清单核细胞趋化蛋白-1(monocyte chemoattractant protein-1;MCP-1)与下肢大血管病变的相关性。方法:(1)2型糖尿病患者61例,根据是否合并下肢大血管病变,分成无下肢大血管病变组(30例)、合并下肢大血管病变组(31例)与正常对照组(20例)对比,用双抗体夹心ELISA法测出血清MCP-1,比较组间血清MCP-1水平的异常。(2)测出各组甘油三酯、胆固醇、低密度脂蛋白、高密度脂蛋白、空腹血糖、糖化血红蛋白、纤维蛋白原等水平,分析各指标和2型糖尿病大血管病变的相关性。结果:(1)2型糖尿病组血清MCP-1水平明显高于正常对照组(P<0.05),合并下肢大血管病变组血清MCP-1水平明显高于无下肢大血管病变组和正常对照组(P<0.05),(2)以T2DM组为整体,有无下肢大血管病变为因变量Y(有=1,无=0),用MCP-1等其它危险因素为自变量,Logstic回归分析,病程、收缩压和MCP-1入回归方程。结论:MCP-1可能是T2DM下肢大血管病变的一个重要的独立危险因素。
Objective:To assess the association of circulating levels of MCP-1 and type 2 diabetes mellitus with/without macroangiopathy of lower limb. Methods:(1) To measure the plasma levels of MCP-1 by ELISA in 61 type 2 diabetic patients with or without lower limb blood vessels and 20 normal controls. the difference between the three groups was compared. The serum concentra-tion of TG,TC,LDL,HDL-ch,FPG,HbA1c,FIB was determined in the three groups,in order to analyze their relationship with macroangiopathy of lower limb in type 2 diabetics. Results:(1) The plasma level of MCP-1 in type 2 diabetics was obviously higher than that of controls(P0.05) . The plasma level of MCP-1 in type 2 diabetics with lower limb macroangiopathy was higher than in the other two groups(P0.05) .(2) The multiple stepwise logistic regression analysis was used to define the association between serum MCP-1 and macroangiopathy of lower limb. Systolic hypertension,course and serum MCP-1 were related significantly with macroangiopathy of lower limb in type 2 diabetics. Conclusions:Possibly MCP-1 is one of the most important independent risk factors for type 2.
