穿透性角膜移植术后外伤性伤口破裂临床观察

Traumatic wound dehiscence after penetrating keratoplasty

目的观察穿透性角膜移植（ＰＫＰ）术后外伤性伤口裂开的临床表现、原因及预后。方法回顾性收集７年中所遇ＰＫＰ术后外伤性伤口破裂１２例（１２只眼）。其中眼球钝挫伤７只眼，头部摔伤３只眼，其它２只眼。结果伤口裂开无固定位置，但与受伤部位及受伤方式有关；预后与伤口裂开范围及眼内容物脱出的多少及伤口的修复有关。手术结果５只眼植片维持透明或半透明，３只眼混浊，４只眼眼球萎缩。结论外伤性伤口破裂是ＰＫＰ术后严重的并发症之一，不但影响植片的命运，还危及眼球的安全。

ObjectiveTo observe the clinical features,cause and prognosis of traumatic wound dehiscence after penetrating keratoplasty.Methods12 cases (12 eyes) experienced traumatic wound dehiscence after penetrating keratoplasty in last 7 a were studied.ResultsAmong 12 eyes,7 eyes had ocular blunt contusion,3 had head injury and the other 2 eyes showed corneal wound dehiscence.The places of wound dehiscence were different,but it was related to location of trauma and mode of injury.The prognosis was related to the volume loss of intraocular contents,the range and repair of wound dehiscence.Results showed that 5 corneal grafts were transparent or semitransparent,3 eyes were opacified and 4 eyes were atrophic.ConclusionsTraumatic wound dehiscence is a serious complication after penetrating keratoplasty.It not only affects corneal graft but also damages the eyeball,therefore it must treated in time in order to decrease its harmfulness.
