摘要
目的:探讨细胞色素P4501A1(CYPIAl)Ile/Val位点基因多态性与食管癌发生的关系。方法:对国内外1997至2008年采用病例对照方法研究CYPlAlIle/Val基因多态性与食管癌发生关系的14篇文献(累计食管癌病例1158例、对照2154例)进行meta分析,应用显性模型(突变基因型Ile/Val+Val/Val与野生基因型Ile/Ile比较)先进行综合定量分析,再按病理类型(鳞癌/腺癌)分亚组进行分析。结果:综合分析显示携带CYPlAl突变基因型(Ile/Val+Val/Val)的个体发生食管癌的危险性是野生型的1.39倍(95%CI1.07~1.80);亚组分析显示突变基因型与食管鳞癌发生的易感性有关但与食管腺癌无关,OR值分别为1.43(95%c,1.07~1.91)和1.20(95%吖0.62—2.30)。结论:CYPlAlIle/Val位点突变基因型可增加食管鳞癌发生的危险性,但与食管腺癌无关。
Aim: To examine the relationship between the cytoehrome P4501A1 (CYP1AI) Ile/Val gene polymor- phism and occurrence of esophageal carcinoma. Methods:Data from 14 literatures about case-control studies on associa- tion of CYP1 A1 Ile/Val gene polymorphism with esophageal carcinoma risk were analyzed using recta-analysis by domi- nant model [ (Ile/Val + Val/Val)/(Ile/Ile) ]. A total of 1 158 esophageal carcinoma cases and 2 154 controls were in- cluded in the analysis. Results:The association between CYP1A1 Ile/Val and esophageal carcinoma had statistical signif- icance with OR of 1.39 ( 95% CI 1.07 - 1.80). In subgroup-analysis the ORs of CYP1 A1 Ile/Val and esophageal squa- mous cell carcinoma (ESCC),esophageal adenocareinoma (EAC) were 1.43(95% C1 1.07 - 1.91) and 1.20(95% CI O. 62 -2.30) ,respectively. Conclusion: CYP1A1 Ile/Val variant genotype may increase the risk of ESCC, but has no association with EAC.
