摘要
目的:研究脓毒症时心肌损伤与心肌内皮素-1(ET-1)、肿瘤坏死因子-α(TNF-α)表达的关系。方法:24只雄性SD大鼠随机均分为3组,正常对照组、假手术组、脓毒症组,每组8只。采取盲肠结扎穿刺术(CLP)建立脓毒症模型,24h后动脉采血及分离左心室心肌,检测大鼠血清心肌肌钙蛋白I(cTnⅠ)、心肌匀浆上清液中TNF-α、ET-1浓度,并分别观察心肌显微及超微结构改变。结果:脓毒症组大鼠血清cTnⅠ较正常对照组及假手术组明显升高(P<0.01),心肌出现典型变性及超微结构损伤;心肌TNF-α、ET-1较正常对照组及假手术组明显升高(P<0.01)。脓毒症组血清cTnⅠ与心肌TNF-α、ET-1水平正相关(γ值分别为0.967和0.958,P<0.001)。结论:脓毒症时心肌损伤与心肌TNF-α、ET-1水平上调有关。
Objective To investigate the relationships between myocardial tumor necrosis factor-α(TNF-α) and endothelin-1 (ET-1) levels and myocardial injury in rats with sepsis. Methods Twenty-four male SD rats were randomly divided into 3 groups: normal control group (n = 8), sham group (n = 8), and sepsis group (n = 8). Twenty-four hours after the sepsis model had been constructed by cecal ligation and puncture (CLP), myocardium of left ventricle was isolated and blood sample was collected to measure serum level of cardiac troponin Ⅰ (cTn Ⅰ) and levels of TNF-α and ET-1 in supernatant liquid of myocardial homogenate, then the pathological changes of myocardium were observed by light microscope and electromicroscope. Results Serum level of cTn I and myocardial TNF-α and ET-1 were significantly higher in sepsis group than those in normal control group and sham group (P 〈 0.01 ). Obvious myocardial degeneration and uhrastructure injury were presented under microscope and electromicroscope in the sepsis group. Serum level of cTnI was positively related to myocardial TNF-α (r = 0.967, P 〈 0.001) and ET-1 (r = 0.958,P 〈 0.001 ). Conclusion Upregulation of myocardial TNF-oL and ET-1 are related to myocardial injury in rats with sepsis.
