摘要
目的观察兔眼球内异物伤合并视网膜损伤后视觉电生理的变化,并分析引起视力损害的可能原因。方法将射钉打入眼球建立兔眼球内异物伤合并视网膜损伤模型,为观察不同致伤因素对兔眼视功能的影响,特设立阳性对照组,即作巩膜切口,将与实验组相同的射钉植入玻璃体腔;阴性对照组只在巩膜作切口,不植入射钉。按照国际临床视觉电生理学会的标准化方案,各组分别于伤前和伤后1d、3d、1w、2w、4w和8w进行闪光视网膜电图(flash electroretinogram,F-ERG)检查,分析各组maxa波和b波的幅值。结果阴性对照组在伤后1d时maxa波和b波幅值较伤前显著下降(P<0.01),伤后3d便恢复到伤前水平(P>0.05)。阳性对照组在伤后各时间点maxa波和b波幅值均显著低于伤前水平(P<0.01)。实验组伤后各时间点a、b波幅值均显著低于两对照组相同时间点水平(4w时maxa波较阳性对照组P<0.05,其余P<0.01)。结论球内异物伤合并视网膜损伤对视功能有严重影响,其中压力波的损害是主要因素之一。
Objective To observe the visual electrophysiologieal changes after intraoeular foreign body injury combined with retinal injury in rabbits, and to analyze the possible factors influencing the final visual results. Methods The rabbit models of intraoeular foreign body injury combined with retinal injury were established by shooting nails into their two eyes in experimental group. Same shoot nails were implanted into vitreous chamber via a selera incision in positive control group, while animals in negative control group were only made a cut on the selera without implanting shoot nails. According to the standard scheme of International Society for Clinical Eleetrophysiology of Vision,rabbits in each group were examined by flash eleetroretinogram (F - ERG) before injury and at 1 d,3 d,1 w,2 w,4 w,8 w after injury,and then max a - and b - wave amplitude were analyzed in each group. Results Compared with pre - injury ,both max a - wave and b - wave amplitude significantly reduced at 1 d post - injury (P 〈0.01 ), and then recovered at 3 d in negative control group ( P 〉 0.05 ). In positive control group, all of a - wave and b - wave amplitude were significantly lower at each time point after injury than those before injury ( P 〈 0. 01 ). Compared with the two control groups, a - wave and b - wave amplitude significantly in experimental group decreased at each time point post - injury ( P 〈 0.05 or P 〈 0. 01 ). Conclusion Intraoeular foreign body injury combined with retinal injury can severely damage visual function, and pressure wave is one of the important factors to influence the final vision
