舒督补肾方联合西药治疗活动期强直性脊柱炎85例临床观察

Clinical Observation on 85 Patients with Ankylosing Spondylitis at Active Stage Treated by Shudu Bushen Recipe Together with Western Medicine

目的观察舒督补肾方治疗活动期强直性脊柱炎的临床疗效。方法将165例活动期强直性脊柱炎患者随机分为两组,对照组(80例)口服柳氮磺吡啶、醋氯芬酸胶囊,治疗组(85例)在西药基础上加用舒督补肾方,两组均治疗6个月。分别于治疗前后观察两组患者晨僵、关节疼痛情况、关节功能等临床指标,血沉、C-反应蛋白、免疫球蛋白等实验室指标,影像学变化及不良反应。结果治疗组总有效率94.12%,对照组为76.25%,治疗组优于对照组(P<0.05)。除对照组枕-墙距治疗前后无明显变化外,两组患者其他临床指标和实验室指标均较治疗前改善(P<0.05或P<0.01),且治疗组治疗后晨僵时间、外周疼痛关节数、血沉、C-反应蛋白等指标与对照组治疗后比较差异有统计学意义(P<0.05或P<0.01);治疗前后骶髂关节X线变化,两组比较差异无统计学意义(P>0.05);治疗组发生不良反应较对照组少(P<0.05)。结论舒督补肾方联合柳氮磺吡啶、醋氯芬酸胶囊治疗活动期强直性脊柱炎优于西药治疗,且不良反应少。

Objective To observe the clinical therapeutic effect of Shudu Bushen Recipe （Recipe to strengthen the spine and rein- force the kidney） on ankylosing spondylitis at active stage. Methods Totally 165 patients with ankylosing spondylitis at active stage were randomly divided into a treatment group （85 cases） and a control group （80 cases）. Both groups were treated with Sulfasalazine and Aceelofenac, and the trentment group was given Shudu Bushen Recipe additionally. Six-month treatment was one course. The clinical changes such as morning stiffness, joint pain and function, erythroeyte sedimentation rate （ESR）, C-reactive protein （CRP）, immunoglobulin （Ig）, X-ray of sacroiliac joint, and adverse reaction were observed. Results In the treatment group, the total effec- tive rate was 94. 12~, superior to that in the control group （76.25%）, with a significant difference （P^0.05）. Compared with that before treatment, only the occiput-wall distance of the control group remained unchanged, other clinical and laboratory indices of both groups improved （P〈0. 05 or P〈0. 01）. The duration of morning stiffness, the number of peripheral joint pain, ESR, CRP of the treatment group after treatment were significantly different from those of the control group （P d0. 05 or P^0. 01）. There was no sig- nificant difference in the changes of X-ray of sacroiliac joint of both groups （P^0. 05）. The adverse reaction occurred in the treatment group was less than that of the control group （P^0. 05）. Conclusion To treat ankylosing spondylitis at active stage, the treatment with Shudu Bushen Recipe in combination with Sulfasalazine and Aceclofenac was more effective and with less adverse reaction than treatment with western drugs.
