脓毒症患者内皮细胞功能及相关炎症因子与中医证型的相关性

Correlation Between Endotheliocyte Function,Inflammation-Related Factors and TCM Syndromes in Sepsis Patients

目的动态整体观察脓毒症患者内皮细胞功能及相关炎症因子与中医证型的相关性。方法将68例脓毒症患者辨证分为气分组23例、营分组28例、血分组17例,另设对照组26例,分别检测各组患者血清血管性血友病因子(vWf)、一氧化氮(NO)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-2(IL-2)、白细胞介素-4(IL-4)和白细胞计数(WBC)等指标。结果TNF-α、IL-6、WBC各脓毒症组水平均明显高于对照组(P<0.05或P<0.01);IL-4营分组和血分组与对照组相比均出现升高(P<0.01或P<0.05);vWf、NO水平均是血分组高于对照组(P<0.05或P<0.01)。各脓毒症组间比较,NO血分组水平明显高于气分组和营分组(P<0.01);vWf血分组水平明显高于营分组(P<0.01)。结论脓毒症vWf、NO水平随着中医证型的变化而不同,故动态观察vWf、NO水平可作为脓毒症辨证的相关参考指标。

Objective To study dynamically the correlation between endotheliocyte functions, inflammation-related factors and TCM syndromes in sepsis patients. Methods According to the TCM syndrome differentiation, 68 septic patients were divided into Qi-fen group （23 cases）, Ying-fen group （28 cases）, and Xue-fen group （17 cases）. The control group （26 cases） was built up for contrast. Serum von willebrand factor （vWf）, nitric oxide （NO）, tumor necrosis factor-a （TNF-a）, interleukin 6 （IL-6）, interleukin 2（IL-2）, interleukin 4 （IL-4）, and white blood cell （WBC） count of all groups were measured. Results TNF-a, IL-6, and WBC in all the sepsis groups were significantly higher than those in the control group （P〈0. 05 or P〈0. 01）. IL-4 in the Ying-fen group and Xue-fen groups was increased （P〈0. 01 or P〈0. 05）. The vWf and NO in the Xue-fen group were higher than those in the control group （P〈0. 05 or P〈0.01）. Comparison in the sepsis groups showed that NO in the Xue-fen group was significantly higher than that in the Qi-fen group and Ying-fen groups （P^0.01）; while vWf in the Xue-fen group was significantly higher than that in the Ying-fen group （P^0.01）. Conclusion The level of vWf and NO of sepsis patients varies with the different TCM Syndromes. Dy- namic observation on vWf and NO contributes to the TCM syndrome differentiation for sepsis.
