山东省颅脑创伤患者住院费用及影响因素路径分析

Path Analysis on Determinants of Traumatic Brain Injury Inpatient Cost in Shandong Province

目的:了解山东省颅脑创伤患者的住院费用情况,并分析其影响因素,为有效降低颅脑创伤患者的经济负担提供依据。方法:对山东省20家医院4832例颅脑创伤病例填写"华东六省一市颅脑创伤流行病学调查表",对住院费用进行描述性分析,利用路径分析方法探讨其影响因素。结果:2004年山东地区颅脑创伤患者人均住院费用为9852.3元,中位数为5185.4元。住院日、职业对住院费用有直接影响,外伤原因、颅脑创伤严重程度、颅脑创伤类型、医院级别、支付方式对住院费用有直接影响和间接影响。结论:通过加强院前救治、缩短住院时间、提高医疗保险监管力度等措施能有效减少住院费用。

Objective：To investigate the hospital costs and its related factors of traumatic brain injury（TBI） patients in Shandong Province,and to provide references for reducing the TBI patient＇s economic burden in future.Methods：The 4832 inpatients in 20 hospitals filled in survey tables named ＂An investigation of hospitalized patients with traumatic brain injury in eastern china in 2004＂.The statistics were analyzed by descriptive analysis and path analysis.Result：The average hospital cost per case was 9852.3 yuan and the median was 5158.4 yuan in 2004.The path analysis demonstrated hospital days and careers had direct effects on inpatient cost,injury reasons,hospital types,injury types,injury grades and nature of expense had both direct and indirect effects on inpatient cost.Conclusions：Inpatient cost could be effectively lightened by taking strategies,such as enhancing the care and treatment before hospitalization,shortening the duration of hospitalization,supervising expenses of health insurance and so on.
