背阔肌皮瓣修复四肢软组织缺损的临床应用

Clinical application of repairing limbs soft tissue with free latissimus dor simuscle-skin flap

目的:探讨背阔肌皮瓣游离移植修复四肢复杂组织缺损合并感染或骨外露创面的临床效果。方法:2003年3月～2009年8月,应用背阔肌皮瓣游离移植治疗四肢复杂组织缺损合并感染或骨外露创面15例。结果:15例皮瓣全部成活,创面一期愈合,经3个月～6年随访,皮瓣成活良好,所修复肢体恢复良好的外形与功能。结论:背阔肌皮瓣部位隐蔽,血运丰富,切取范围大,肌瓣可填塞死腔,修复肌肉缺损,抗感染能力强,是修复四肢复杂组织缺损合并感染或骨外露创面的理想皮瓣。

Objective To explore the clinical result of repairing complicated soft tissue and bone defects around limbs with free latissimus dorsi muscle-skin flap.Methods 15 cases of limbs soft tissue and bone defects were treated with free latissimus dorsi muscle-skin flap from March 2003 to August 2009.Results All flaps survived completely,the wound healed in the initial treatment,ranged from 3 months to 6 years follow-up study revealed the flaps were in good condition and the limbs recovered to satisfactory appearance and useful function.Conclusion Free latissimus dorsi muscle-skin flap has such advantages as covert part,adequate blood supply,big dermatomic area and excellent anti- infective ability,which is an ideal flap for repairing complicated soft tissue and bone defects around limbs.
