呃逆患者68例的病因及治疗分析

Analysis on Etiology and Treatment of 68 Patients with Hiccup

目的研究呃逆原因及治疗方法,提高对呃逆的认识。方法回顾性分析武汉大学人民医院自2008年9月—2009年9月收治的68例呃逆患者的临床资料,分析其病因,将病人随机分为A组(23例)、B组(22例)、C组(23例)。在积极治疗原发病的同时,A组接受氯丙嗪12.5mg肌内注射,2次/d;B组接受针灸治疗,1次/d,3d为1疗程;C组接受氯丙嗪12.5mg肌内注射,2次/d,再加用针灸治疗。结果68例呃逆患者中,消化系统疾病21例(30.88%)、脑血管疾病23例(33.82%)、心血管疾病6例(8.82%)、呼吸系统疾病7例(10.29%)、药物引起者4例(5.88%)、功能性呃逆7例(10.29%),引起呃逆的病因以消化系统及脑血管疾病居多。3种治疗方案治疗呃逆有效率存在差异(P<0.05),C组治疗方案的有效率最高(91.30%),A组治疗方案有效率最低(60.87%)。结论引起呃逆的病因众多,但以消化系统及脑血管疾病居多。在使用氯丙嗪的基础上,加用针灸治疗,呃逆的治愈率大大提高。

[Objective]To study the etiology and treatment of hiccup in order to enhance the understanding of it.[Methods] The clinical data of 68 patients with hiccup from September 2008 to September 2009 were analyzed for the etiology. All patients were randomly divided into group A （23cases）,group B （22cases） and group C （23cases）. When treated the protopathy,the patients in group A were treated by Chlorpromazine intramuscular injection of 12.5mg,2 times / day; group B received acupuncture,1 time / day,3 days for a course of treatment; group C received Chlorpromazine intramuscular injection 12.5mg ,2 times / day,plus acupuncture.[Results]Among 68 patients,digestive system diseases were in 21（30.88%）,cerebrovascular diseases were in 23（33.82%）,cardiovascular diseases were in 6 （8.82%）,respiratory diseases were in 7（10.29%）,drug-induced hiccup were in 4（5.88%）,functional hiccup were in 7（10.29%）. The digestive system and cerebral vascular diseases were the main causes of hiccup. There were differences among 3 therapies （P0.05）. The therapy received by group C had the highest efficiency （91.30%）,the therapy received by group A the lowest efficiency （60.87%）.[Conclusion]Hiccup can be caused by many diseases,but the main causes are digestive system and cerebral vascular diseases. When in application of Chlorpromazine,with the acupuncture added,the cure rate of hiccup can increase significantly.
