64层CT对活体肾移植供体术前应用价值 被引量:2

The value of 64-slice CT in the preoperative evaluation of living renal donor

摘要 目的:探讨64层螺旋CT在活体肾移植供体术前临床应用价值。方法:60例活体亲属供肾者术前1周内行64层CT检查,所有供者均采用平扫及动脉期、静脉期、排泄期四步扫描法进行扫描。图像处理采用CPR,MIP及3DVR重建。将CT表现与经手术证实的56名供者术中所见进行对照,评价64层CT在术前评估供肾血管、尿路及肾实质情况的价值。结果:56名手术证实除1例副肾动脉CTA漏诊以外其余动静脉主干、副肾动脉、动脉提前分支,肾静脉主干变异及其属支,尿路及肾实质情况显示与术中一致。结论:64层CT可直观显示活体供肾者肾血管、尿路及肾实质生理及病理改变,对活体供肾者术前综合评估有重要临床价值。 Objective:The purpose of this study was to evaluate the value of 64-slice CT in the preoperativ evaluation of living renal donors.Methods:Sixty living renal donors underwent four-section(unenhanced,arterial phase,venous and excretory phase) 64-slice CT scanning before operation.Image processing included curved planar reformation(CPR),maximum intensity projection(MIP),and 3D volume rendering(3DVR).All the findings of CT were compared with intraoperative findings for 56 donors,so as to investigate the value of 64-slice CT in assessing renal vascularity,urinary tract and lesinons of renal parenchyma.Results:The CTA findings of the 56 donors were in accordance with intraoperative findings in demonstrating the anatomy of renal arteries and renal viens,accesary arteries,early branching of renal artery,urinary tract and renal parenchyma except one accesary artery.Conclusion:64-slice CT can directly and correctly show the details of renal vascularity,urinary tract and renal parenchyma and it has high value in the evaluation of living renal donor before transplantation.
