实时三维超声心动图测量左室重构患者左室容积的评价 被引量:4

Evaluation of accuracy of real time three dimensional echocardiography in quantifying left ventricular volume in patients with left ventricular remolding

摘要 目的:评价实时三维超声心动图(RT3DE)测量左室重构患者左室容积的准确性。方法:对17例健康体检者和15例左室重构患者,应用实时三维超声诊断仪采集心尖长轴左室全容积图,脱机后应用容积分析软件测量左室舒张末期容积(LVEDV),同时应用常规二维超声心动图Simpson′s法和M型超声心动图法测量LVEDV。测值分别与多层螺旋CT(MSCT)结果进行对照分析。结果:正常组:LVEDV测值:RT3DE法、Simpson′s法及M型法与MSCT之间的相关性r分别为0.938、0.854、0.760(P均<0.05);左室重构组:LVEDV测值:RT3DE法、Simpson′s法及M型法与MSCT之间的相关性r分别为0.892、0.795、0.716(P均<0.05)。RT3DE法与MSCT测值差显著小于Simpson′s法及M型法(P<0.05)与MSCT的测值差.同一观察者及两位观察者间RT3D测量LVEDV的差异无显著性,且具有高度的可重复性(P<0.05)。结论:实时三维超声心动图可以准确测量左室重构患者的左室容积,其准确性要高于传统超声心动图,可重复性好。 Objective: To assess the accuracy of real time three dimensional cchocardiography (RT3DE) in quantifying left ventricular volume in patients with left ventricular remolding. Methods.. RT3DE was performed in 17 normal subjects (normal group) and 15 patients with left ventricular remolding (left ventricular remolding group). The left ventricular end-diastolic volume (LVEDV) was measured off-line using analysis software, by two-dimensional Simpson's method and M- mode method. The values of LVEDV were compared with Multi-slice computed tomography (MSCT) respectively. Results: LVEDV measured by RT3DE, Simpsonts method and M-mode methods was both correlated well with MSCT in normal subjects group and left ventricular remolding group (normal subjects group: r= 0. 938, 0. 854, 0. 760 ; left ventricular remolding group., r = 0. 892, 0. 795, 0. 716), P〈0. 05 all. The difference between RT3DE amd MSCT was less than that of mong Simpson's, M-mode methods and MSCT (P〈0. 05). For examination of LVEDV by RT3DE, there was no difference between two examinations of one observer and two observers in two groups (P〉0.05). Conclusion; RT3DE can accurately measure left ventricular volume in normal and left ventricular remolding patients. The accuracy of RT3DE is higher than that of traditional echocardiography and its repeatability is excellent.
