改进的结膜瓣转位术治疗翼状胬肉的临床观察

Microsurgical treatment of pterygium by excision combined with autologous Tongue-shaped bulbar conjunctiva transposition

目的评价改进翼状胬肉的手术方法降低复发率的效果。方法原发性翼状胬肉60例（78眼），在手术显微镜下广泛切除变性的眼球筋膜囊组织联合自体舌形球结膜瓣转位修复方法，以最小的手术损伤，尽可能修复角膜缘上皮（干细胞）的屏障功能。结果术后随访12～24个月，平均18个月，2眼复发（2．56％），视力明显提高。结论作者设计的改进显微手术切除联合舌形球结膜瓣转位术，手术操作简单，术后复发率低。

Objective To improve the surgical method of Pterygium, so as to reduce the rate of recurrence. Methods 78 eyes from 60 patients with the primary pterygium participated in the study Extensive excision of the degenerated Tenon＇ s capsule using an operating microscope combined with autologous Tongue-shaped bulbar conjunctiva transposition is applied, which reduced the degree of surgical injury by restoring the protective function of the epithelial of Limbal cornea （the stem cell）. Results Witha 12- month to 24-month follow-up （ the mean month is 18） , the pterygium recurred in 2 eyes, and tiao recurrence rate is only 2.6% （2/ 78）. The eyesight of the patients was improved obviously. Conclusion In this study, microsurgical excision of pterygium Combined with autologous Tongue-shaped bulhar conjunctiva transposition was comfirmed to be an easy procedure to perform, with a lower recurrence rate. This surgical technique remarkably improved the eyesight and patients＇ respose to the operation was slight. All of these make tlnis technique worthy of clinical application as early as possible.
