cN0期甲状腺微小乳头状癌颈中央区淋巴结清扫54例分析

The analysis of prophylactic central region cervical lymph node dissection in stage cNO papillary thyroid microcarcinoma （ report of 54 cases）

目的探讨cN0期甲状腺微小乳头状癌预防性颈中央区淋巴结清扫的价值。方法对54例cN0期甲状腺微小乳头状癌作回顾分析。结果48例行单侧腺叶＋峡部切除，清扫同侧中央区淋巴结；全甲状腺切除6例，清扫双侧中央区淋巴结。13例单侧中央区淋巴结转移（24．1％），术后并发暂时性喉返神经麻痹3例，暂时眭甲状旁腺功能减退2例，治疗后恢复正常，并发症发生率9．3％。结论cN0期甲状腺微小乳头状癌中央区淋巴结转移率高，预防性清扫中央区淋巴结是必要的。

Objective To evaluate prophylactic central lymph node （CLN） dissection tor patient with cN0 papillary thyroid microcarcinoma （PTMC）. Methods 54 patients of oN0 PTMC were retrospective re- viewed. Results Of the 54 patients, 48 patients underwent lobeetomy with isthmectomy and ipsilateral CLN dissection, while 6 patients received total thyroideetomy with bilateral CLN excision. The total rates of CLN metasta- sis was 24.1% （13/54）. The occurrence rates of complication was 9.3% （5/54）. Included 3 cases of temporary recurrent laryngeal nerve paralysis and 2 cases of temporary hypoparathyroidism. Conclusions Metastasis in CLN of cN0 PTMC are relatively common, it＇s necessary to perform prophylactic CLN dissection.
