改良钻颅血肿抽吸治疗高血压急性脑出血34例临床分析与护理

目的:探讨应用改良微创钻颅置管血肿抽吸治疗高血压急性脑出血的临床疗效与护理。方法:将符合入选标准的高血压急性脑出血患者67例随机分为实验组34例和对照组33例,根据出血量将实验组与对照组分别分为2个亚组,出血量50～80ml者分别为Ⅰa组和Ⅱa组,出血量﹥80ml者分别为Ⅰb和Ⅱb组。实验组用改制的上面有可调节钻入深度的限位器钻头钻穿颅骨,置入硅胶管抽吸部分血肿后再引流;对照组给予脑出血常规治疗与护理。治疗与护理后1个月进行疗效评定。结果:改良微创钻颅置管血肿抽吸治疗护理出血量在Ⅰa组的疗效显著优于Ⅰb组,Ⅱa组疗效评定显著进步和进步的患者例数明显多于Ⅱb组,但无统计学意义。结论:改良微创钻颅置管血肿抽吸治疗护理高血压急性脑出血效果显著、方法简单、费用低,术后血肿扩大发生率低。

Objective：To evaluate the improvement of minimally invasive drilling cranial hematoma aspiration catheter treatment of hypertensive effect of acute cerebral hemorrhage and nursing.Methods：Candidates will meet the standard of hypertension in patients with acute cerebral hemorrhage 67 cases were randomly divided into an experimental group of 34 cases and the control group 33 cases,according to the amount of bleeding the experimental and control groups were divided into two sub-groups,the amount of bleeding and the respective 50-80 ml was Ⅰa group and Ⅱa group,bleeding volume＆gt;〉80 ml were respectively Ⅰb and Ⅱb group.Experimental group has restructured the top of the depth limiter adjustable drill into drill bit drilling the skull,placed part of the hematoma after the silicone tube suction drainage;control group was given conventional treatment and care of cerebral hemorrhage.Treatment and care for 1 month after assessment of efficacy.Results：Modified minimally invasive drilling cranial catheterization hematoma suction curative care bleeding volume Ⅰa group efficacy significantly superior Ⅰb group,Ⅱa group efficacy evaluation remarkable progress and progress patients cases were obviously than Ⅱ b group,but no statistical significance.Conclusion：Modified minimally invasive drilling cranial hematoma aspiration home treatment and care of acute hypertensive intracerebral hemorrhage results are obvious,method is simple,low cost,a lower incidence of postoperative hematoma enlargement.
