摘要
目的:研究Propeller LAVA(Liver Accqusition with Volume Acceleration)肝脏三维容积快速多期动态增强扫描对壶腹周围癌的应用价值。材料与方法:应用Propeller LAVA对21例疑壶腹周围癌的病人进行十二指肠低张后增强及延时扫描,应用工作站对所得数据行三维处理,并结合其MRCP和FIESTA(Fast Imaging EmployingSteady-state Accquisition采用稳态采集的快速图像)采集的图像,以3级评分法,判断Propeller LAVA增强扫描对壶腹周围癌直接征象及其临床分型。结果:21例行Propeller LAVA增强扫描者有20例显示肿瘤的直接征象及累及范围,MRCP及FIESTA序列分别有0例和2例显示肿瘤的直接征象及累及范围,三者差异具有统计学意义。(P<0.05)Propeller LAVA增强扫描根据不同类型可表现为肿瘤强化及延迟强化。结论:Propeller LAVA增强扫描能显示壶腹周围癌的直接征象及累及范围,在壶腹周围癌的鉴别诊断上明显优于MRCP和FIESTA序列。
Purpose： To study the value of Propeller LAVA in the diagnosis of periampuUary carcinomas. Materials and Methods： 21 patients with suspected periampullary carcinomas underwent MRI examination after duodenum was in hypo- tonicity. MRI was performed with LAVA dynamic multi - phase contrast enhancement and delayed enhancement, MR cholan- gio- pancreatography（MRCP） pulse sequence and fast imaging employing steady -state accquision （FIESTA）scans. Com- bined with the MRCP and FIESTA images, the reconstructed images of Propeller LAVA sequence were graded by 3 levels （ranks） to evaluate the value and clinical types of Propeller LAVA dynamic contrast sequence on the direct charateristics of periampullary carcinomas. Results： In 21 cases, there were 20 cases demonstrated the direct characteristics and the exten- sion of the tumors by Propeller LAVA sequence while 0 and 2 cases by MRCP and FIESTA, respectively. There was signfi- cant difference of demonstration ability between Propeller LAVA sequence and MRCP or FIESTA （ P 〈 0. 05 ）. The tumors were graded into enhancement and delay enhancement according to different types by Propeller LAVA contrast sequence. Conclusion： Propeller LAVA contrast sequence images can demonstrate the direct characteristics and extension of periampul- lary carcinomas and may be better than MRCP and FIESTA sequences on differentiating diagnosis of periampullary carcinomas.
