摘要
目的:运用肠易激综合征(Irritable bowel syndrome,IBS)大鼠模型,观察针刺上巨虚穴对IBS大鼠模型胃肠道5-羟色胺(5-hydroxytryptamine,5-HT)分布的影响及其与腹部撤回反射(abdominal withdrawal reflex,AWR)评分变化的相关性,研究针刺的作用规律,为临床治疗提供科学依据。探讨电针上巨虚穴缓减内脏高敏感的可能机制及其不同电针次数的效能。方法:雄性3周龄50只SD大鼠,按体重随机分为5组:空白组、模型组、多次电针组、单次电针组、假针组。每组10只。除空白组外其余各组大鼠每天接受结直肠扩张刺激(colorectal distention CRD),总共持续刺激2周后开始针刺双侧上巨虚穴。对大鼠CRD诱发刺激下的AWR进行半定量评分,运用免疫组化SABC法观察大鼠结肠5-HT分布。结果:多次电针在电针后90min内可以明显的降低内脏痛;单次电针、单次针刺在治疗后90min内仅在20mmHg下有效。与多次电针组比较:模型组大鼠5-HT表达明显降低(P<0.01);单次电针组、单次针刺组大鼠5-HT表达明显降低(P<0.01);单次电针较单次针刺组大鼠5-HT表达无显著性差异(P>0.05)。各治疗组AWR评分的变化与结肠5-HT变化相一致。结论:电针上巨虚穴能降低结肠中VIP的阳性反应物含量,且表现多次电针效果优于单次电针、单次假针,与AWR评分结果相吻合。电针大肠腑下合穴上巨虚对IBS有一定的治疗作用,其作用机制可能降低5-HT的含量有关。
Objective: With the model of irritable bowel syndrome (IBS), we observed the influence of needling Shang -ju -xu on 5 - HT in rat' s gastrointestinal tract and the relationship between needling Shang -ju - xu and the score of abdominal withdrawal reflex (AWR) to study the role of acupuncture laws and provide a scientific basis for clinical treatment. We also investigated the possible mechanism of electro - acupuncture in Shang - ju - xu and the effectiveness of different acupuncture times. Method : 50 three - week - old male SD rats were randomly divided into five groups: blank control group, model group, multiple electroacupuncture group, a single electro- acupuncture group, and sham acupuncture group. Each group included 10 rats. Except for the rats in blank control group, the other ones in the four groups accepted the stimulation of colorectal and rectal distention (CRD) for two weeks. Then they accepted acupuncture on bilateral Shang -ju - xu. Semiquantitative grading was done on AWR induced by CRD and the distribution of 5 - HT in rat' s colon was observed with the method of immunohistochemistry SABC. Result: In multiple electro - acupuncture group visceral pain was reduced markedly within 90 minutes after electro - acupuncture, but in single electro - acupuncture group and single acupuncture group (sham acupuncture group) the efficacy showed only in 20mmHg within 90 minutes after operation. Compared with multiple electro - acupuncture group, 5 - HT was decreased markedly in model group (P 〈 0.01 ), and so did in single electroacupuncture group and single acupuncture group (sham acupuncture group) (P 〈 0.01 ). But 5 - HT didn' t show significant difference between single electro - acupuncture group and single acupuncture group ( sham acupuncture group) (P 〉 0.05). The score of AWR in each group was the same as 5 - HT. Conclusion: Acupuncture on Shang-ju -xu can reduce VIP's positive reactant in colon, and the efficacy of multiple electro -acupuncture is better than single electro -acupuncture sham acupuncture. This is coincident to the score of AWR. Electro - acupuncture on Shang -ju - xu has a therapeutic effect on treating IBS, and its active mechanism is related to reducing 5 - HT.
