丹参酮ⅡA磺酸钠注射液致严重过敏反应2例

Two cases of serious anaphylaxis induced by sodium tanshinone Ⅱ A sulfonate injection

2名男性患者给予丹参酮ⅡA磺酸钠注射液治疗时出现头晕、头痛、胸闷、全身发麻、高热、斑丘疹等不良反应。病例1,1例60a男性患者,因车祸致伤入院。术后给予丹参酮ⅡA磺酸钠注射液8mL+0.9%氯化钠注射液500mL静脉滴注,输液约3h时患者出现头晕、头痛、胸闷、全身发麻,立刻停止滴注,并给予地塞米松磷酸钠进行抗过敏治疗,20min后症状逐渐好转。遂改用血栓通注射液,未再发生上述不良反应。病例2,1例43a男性患者,因车祸致伤入院。术后给予丹参酮ⅡA磺酸钠注射液8mL+5%葡萄糖注射液500mL静脉续滴,每日1次。次日继续静滴,10min后出现丘疹,6h后出现高热,体温39.6℃,伴斑丘疹。立刻停止滴注,并给予物理降温及肌内注射复方氨林巴比妥,5min后体温39.7℃。给予口服氯雷他定片,并静脉滴注地塞米松磷酸钠,10min后体温38.2℃,但斑丘疹未见明显消退。用药3d后基本恢复。

Two men receiving sodium tanshinoneⅡA sulfonate injection developed dizziness,headache,dyspnea,body numb,high fever,maculopapular rash and other adverse reactions.Patient 1,a 60-year-old male patient was hospitalized due to accident injuries.He was given intravenously sodium tanshinone ⅡA sulfonate injection 8 mL and 500 mL of 0.9% sodium chloride injection after operation.About three hours later,the patient developed dizziness,headache,chest tightness,body numb.After discontinuation of infusion instantly,he received dexamethasone sodium phosphate for anti-allergy treatment.Twenty minutes later,the symptoms of patient improved.The sodium tanshinoneⅡA sulfonate was replaced with Xueshuantong injection.No such adverse reactions occurred.Patient 2,a 43-year-old male was hospitalized due to accident injuries.He was given intravenously sodium tanshinoneⅡA sulfonate injection 8 mL and 500 mL of 5% glucose injection once daily after operation.The next day,Ten minutes after infusion he developed pimple,and six hours later he suffered with high fever（T 39.6 ℃）.After discontinuation of infusion instantly,he was given physical cooling and an intramuscular injection of compound aminopyrine barbital.But five minutes later,his temperature didn＇t reduce（T 39.7 ℃）.Then he was given loratadine tablets orally and dexamethasone sodium phosphate by intravenous infusion.Ten minutes later,the temperature reduced to 38.2 ℃,but the maculopapular rash didn＇t subside obviously.He almost restored after three days.
