痛点注射治疗肩胛背神经卡压综合征128例疗效分析

Clinical analysis of 128 patients with dorsal scapular nerve compression syndrome treated with painpoint injections

目的探讨痛点注射治疗肩胛背神经卡压综合征的临床疗效。方法采用颈部压痛点与胸3、4棘突旁压痛点注射治疗肩胛背神经卡压综合征患者128例，于末次注射治疗后半年～1年门诊或电话随访进行疗效评价。结果所有患者在接受治疗后数分钟内颈肩背部酸痛症状消失或明显缓解。随访期间，优效87例，良效28例，差效3例，无效2例，1例治疗后明显缓解，3个月后症状加重，再次给予注射1次后症状消失，7例失访。疗效与性别及年龄无关（P〉0．05）；病程≤半年者较病程〉半年者疗效佳（P〈0．05）；注射次数1～3次者较注射次数4～6次者疗效佳（P〈0．05），需要注射4～6次者有90．9％病程〉半年，而需要注射1～3次者仅有33．6％病程〉半年（P〈0．05）。结论痛点注射是治疗肩胛背神经卡压综合征的有效方法。

Objective To investigate the clinical efficacy of pain-point injection on dorsal scapular nerve compression syndrome. Methods One hundred and twenty-eight patients with dorsal scapular nerve compression syndrome were treated with pain-point injections at tender points of the neck and beside the 3rd and 4th thoracic spinous processes. Clinical efficacy was evaluated after six months to one year of the last treatments at clinics or by telephone follow-up. Results The pain of the back, neck and shoulder in all patients disappeared or significantly alleviated within several minutes after receiving treatments. During follow-up period, excellent effect was in eighty-seven cases, good effect in twenty-eight cases, poor effect in three cases, ineffect in two cases, the symptoms of one case was significantly relieved after treatment, but increased after three months, and disappeared after giving the injection again. Seven cases lost to follow up. Gender and age did not influenced on the efficacy（P〉0.05）, the efficacy of patients with the disease duration less than six months was better than that with the disease duration more than six months（P〈0. 05） ; The efficacy of the patients who were treated with one to three injections was better than that with four to six injections（P〈0.05）. 90.9% of the patients treated with four to six injections and 33.6% of the patients with one to three injections had the disease duration of more than six months（P〈0.05）. Conclusion The pain point injection is an effective method to treat dorsal scapular nerve compression syndrome.
