B型钠尿肽对脓毒性休克患者的诊断及预后评估价值

B-type Natriuretic Peptide Used as a Marker for Prognosis of Septic Shock

目的观察B型钠尿肽(BNP)的水平与脓毒症严重性的关系。方法选择脓毒性休克12例,早期脓毒症17例和健康对照组17例。排除其他病态状况(充血性心力衰竭或肾衰竭)的脓毒性休克患者。结果和另外两组相比,脓毒性休克患者BNP水平明显增高(P<0.05)。早期脓毒症患者BNP水平没有明显升高。脓毒性休克患者BNP水平与连续性器官衰竭(SOFA)评分存在正相关。结论研究证实BNP水平与脓毒症患者的心功能及预后有关系,BNP水平可以作为脓毒性休克预后评估的标志物。

Objective To explore the relationship between BNP levels and the severity of septic shock.Methods Twelve patients with septic shock,17 with early sepsis,17 healthy subjects,excluding septic shock patients with other pathological conditions （congestive heart failure or kidney failure）.Sepsis severity was determined by Sequential Organ Failure Assessment scoring system.Patients with sepsis were followed up for 21 days.Serum BNP levels were determined.Results Patients with septic shock had significantly high BNP levels on admission as compared with the other 2 groups （P〈0.05）.The BNP levels did not significantly elevated in patients with early sepsis.Plasma BNP levels were positively correlated with Sequential Organ Failure Assessment scores in patients with septic shock.Conclusion BNP levels,related to myocardial function and prognosis in sepsis patients,can be used as an evaluation marker for septic shock prognosis.
