心肌梗塞左室舒张与充盈动力学参数的无创性测定 被引量:4

NONINVASIVE ASSESSMENT OF LEFT VENTRICULAR RELAXATION AND FILLING DYNAMICS IN PATIENTS WITH MYOCARDIAL INFARCTION

摘要 应用脉冲多普勒超声心动图,对30例心肌梗塞患者和30例正常人左室舒张与充盈动力学参数进行了测定.结果表明,与正常人相比,心肌梗塞患者左室等容舒张时间、舒张早期快速充盈加速和减速时间显著延长;左室舒张早期最大流速、E/A比值、舒张早期充盈分数、舒张早期快速充盈加速度和减速度显著减小;而左室舒张晚期最大流速无显著变化.提示心肌梗塞后左室舒张和充盈功能失常,表现为左室等容舒张延缓和舒张早期快速充盈减少,且E/A比值减小是左室舒张早期最大流速减小所致. Parameters of left ventricular relaxation and filling dynamics were measured by pulsed Doppler echocardiography in 30 patients with myocardial infarction. Isovolumic relaxation time, early diastolic acceleration and deceleration times increased significantly, peak early diastolic velocity, E/A ratio, early diastolic filling fraction, early diastolic acceleration and deceleration rates decreased significantly, but peak late diastolic velocity didn't change, when compared with that in 30 age-matched normal subjects. Thus, the left ventricular relaxation and filling were impaired in patients with myocardial infarction and the characteristics of impairment was prolongation of isovolumic relaxation and decrease of early diastolic filling. The decrease in E/A ratio was the result of the decrease of the peak easly diastolic velocity.
