尿道板双侧旁正中纵切卷管尿道成形术

Tubularized double paramedian incision plate urethroplasty

目的总结尿道板双侧旁正中纵切卷管尿道成形术治疗尿道下裂的经验。方法2008年6月至2009年5月应用尿道板双侧旁正中纵切卷管尿道成形术治疗尿道下裂31例，其中阴茎体型28例，阴茎阴囊型3例。本组平均年龄2．7岁（1．6～5．2岁），平均手术时间91．6min（80～105min）。结果本组病例均在术后随访超过3个月，外观满意，多数病例形似包皮环切术后阴茎外观。尿道外口位于龟头尿道沟正位，裂隙状。术后尿道瘘3例，发生率为9．7%；尿道口狭窄2例，发生率为6．5％，均在尿道扩张后缓解；没有发生尿道狭窄。结论尿道板双侧旁正中纵切卷管尿道成形术治疗尿道下裂效果满意，术后并发症发生率较低，且容易开展。

Objective To present the experience of hypospadias repair using the tubularized double paramedian incision plate urethroplasty. Methods A retrospective review of the last 1 year of tubularized double paramedian incision plate urethroplasty was performed. In 31 patients who had the operations using this procedure, 28 suffered from penile and 3 penoscrotal hypospadias. The average age at surgery was 2. 7 years（range： 1.6 to 5.2 years）. The mean operative time was 91.6 rain （range： 80 to 105 rain）. Results All patients were followed up for more than three months after surgery. Fistulas occurred in 3 patients （9. 7%）. Meatal stenosis occurred in 2 patients （6. 5 % ）. Both responded well to urethral dilatation. No patient had urethral stricture. Conclusions Tubularized double paramedian incision plate urethroplasty is an effective treatment for hypospadias. The incidence of postoperative complications is low.
