摘要 目的:探讨早期判断急性胰腺炎严重程度的简便实用方法.方法:对1995~1996年收治的108例急性胰腺炎病例作回顾性分析.结果:体温≥38℃、血糖≥11.2mmol/L、血钙≤2mmol/L及胰外侵犯的出现可组成重症胰腺炎的预测体系,每一阳性指标计1分,≥2分者列为重症,其在本研究中的敏感性、特异性及准确性分别达95%、89.47%和91.53%.结论:该体系结合了急性胰腺炎的临床生化和形态学特点,较为简便、实用,入院早期即可作出判断. Objective: To search for a simple and effective method of assessing the severity of acute pancreatitis. Methods: 108 cases of acute pancreatitis accepted during 1995-1996 in Ruijin Hospital were analyzed retrospectively. Results: Body temperature≥38℃, serum glucose>11.2mmol/L, serum calcium ≤2mmol/L and extrapancreatic involvements were selected to be prognostic criteria in predicting severity. The presence of two or more of the above-mentioned factors was considered to be an indication of severe pancreatitis; the sensitivity, specificity and accuracy of the method were 95%, 89.47% and 91.53 % respectively. Conclusions: The proposed method of assessment is simple and useful for predicting the severity of early acute pancreatitis.
