重组人红细胞生成素治疗肿瘤化疗相关性贫血388例

Evaluation of recombinant human erythropoietin in the treatment for 388 cases with chemotherapyrelated anemia

目的评价重组人促红细胞生成素(rhEPO)治疗肿瘤化疗相关性贫血的疗效和不良反应。方法 388例肿瘤化疗相关性贫血患者分为观察组192例和对照组196例,对照组给予一般性纠正贫血药物,观察组给予rhEPO肌肉注射。评价2组疗效和副作用。结果对照组治疗后血红蛋白(Hb)和血细胞比容(Hct)呈下降趋势,观察组治疗后Hb和Hct呈上升趋势。治疗4、6和8周后,观察组Hb和Hct均高于对照组,差别有统计学意义(P<0.05,P<0.01)。治疗6周后,观察组生活质量改善率(100.0%)明显高于对照组(21.4%),差别有统计学意义(P<0.01)。治疗后观察组的输血需求率(8.9%)明显低于对照组(25.0%),差别有统计学意义(P<0.01)。2组患者均未出现明显不良反应。结论 rhEPO治疗恶性肿瘤贫血具有一定的疗效,能提高患者的Hb和Hct,有效降低输血需求及风险,且安全性好。

Objective To evaluate the efficacy and side effect of recombinant human erythropoietin （rhEPO） in the treatment for tumor chemotherapy-related anemia. Methods Three hundred and eighty-eight patients with chemotherapy-related anemia were divided into observation group（ n = 192） and control group （n = 196）, rhEPO was injected intramuscularly on pa- tient in the observation group, while patients in the control group were treated only with conventional anti-anemia medicine. Then the efficacy and side effect of the two group were evaluated. Results After treatment, hemoglobin （Hb） level and haematocrit （Hct） level in the patients in the observation group showed a rising trend,while the two index decreased in the control group. Compared with control group, in observation group, H_b and Hct levels were higher after treatment for four weeks, six weeks and eight weeks （ P 〈 0. 05 ,P 〈 0.01 ）, the improvement rate of the quality of life was higher（ P 〈 0.01 ）, the blood transfusion de- mand rate was lower（ P 〈 0.01 ）. No severe side effect was found in two groups. Conclusion rhEPO has a certain effect on ma- lignant carcinoma chemotherapy-related anemia and can improve patient＇s Hb level and Hct level, rhEPO can lower blood trans- fusion requirement and its risks effectively,it has better security.
