晶胶体液扩容预防异丙酚血压下降作用的临床观察

Influrane of Expanding Circulating Blood Volume with Ringer's Lactated Solution or Colloidal Infusion Solution on the Hypotension Following Adminisitration of Propoful

为了观察用晶、胶体扩容对异丙酚血压下降的预防作用，将21例全麻下手术病人，随机分为晶体组、胶体组和对照组（各7例），于麻醉诱导前分别在10～20min内榆入平衡液（晶体组）或代血浆（胶体组）12ml／kg，观察诱导前后的血压和心率变化。结果，三组病人在静脉注射异丙酚后血压均下降，但以对照组下降尤为明显，组间比较，晶、胶扩容组与对照组比较有显著差异（P＜0．05），两扩容组比较则无统计学差异（P＞0．05）。认为，麻醉诱导前用晶体液或胶体液扩容均能在一定程度上防止静注异丙酚后的降血压作用，但用晶体液扩容更符合病人的生理需要。

Twinty - one patients scheduled for general anesthisia were randomly divided into three groups: Ringer'slactated soution guoup, colloidal infusion group and control group. 12ml perkilogrem of Ringer's lactated solution of colloidal infusion were used with intravenois infusion in 10 - 20 minutes before induction, of generalanesthisia MAP and HR change were measired before anesthesia induction, after anesthesia induction. The result showed that hypotension following adminisitration of propoful occured in all patients. But it was lowermarkedly in control group than in other two groups (P<0.05), and there wer no defferent significantly between Riger's lactated solusion group and colloidal infusion group. it is suggested that it's better to use Ringer's lactated solution than colloidal solution as the former is more accord with physiological function
