涎腺良性淋巴上皮病变——附15例临床与病理分析

BLEL of salivary gland——a clinical and pathological analysis of 15 cases

目的：探讨涎腺良性淋巴上皮病变（Ｂｅｎｉｇｎｌｙｍｐｈｏｅｐｉｔｈｅｌｉａｌｌｅｓｉｏｎ，ＢＬＥＬ）与ＥＢ病毒的关系及其诊断要点与治疗手段。材料和方法：自１９６４年以来共收治ＢＬＥＬ１５例，全组病例均经病理标本复习，并采用原位杂交检测ＢＬＥＬ组织内ＥＢ病毒的感染情况；行单纯放疗３例，术后加放疗３例，单纯手术切除８例，单纯药物治疗１例。结果：ＢＬＥＬ组织ＥＢＥＲ１原位杂交检测阳性率为０％，３例单纯放疗患者疗后２、１８、２３年无复发，１例术后８年复发放疗后随访１７年未见再复发。结论：ＥＢ病毒与ＢＬＥＬ在病因学上无相关性；提倡细针穿吸活检；放疗之于本病不失为安全。

Purpose: To invesgate and analyse the relation between benign lymphoepithelial lesion (BLEL) and EB-Virus. To clarify the key factors of diagnosis and the treatment methods. Methods: 15 patients iwth BLEL in salivary gland were treated in our hospital since 1964. We reviewed all the pathologic samples and edtected the infections of EBV in BLEL tissue by ISH. 3 patients underwent radiotherapy only, 3 underwent surgery and postoperative radiotherapy, 8 surgery only, and the other one was administered medicine only. Results: The positive rate of EBER1 tissue by ISH was 0%. No recurrence is found after 2、18 and 23 years in 3 patients who underwent radiotherapy only. One operated patient who recurred after 8 years was given radiotherapy and recurrence is not found afer 17 years follow-up. Conclusion: There is no aetiological relationship between EBV and BLEL. We propose radiotherapy is a safe and effective treatment method.
