胫腓骨骨折患者应用动静脉泵消肿镇痛的效果观察

Effect of AV Impulse Pump on Pain and Swelling-alleviating for Patients with Tibia and Fibula Fracture

目的探讨胫腓骨骨折患者应用动静脉泵消肿及镇痛的效果。方法将69例胫腓骨骨折患者按住院顺序分为观察组33例和对照组34例,观察组在患者入院时至术前1d、术后第1—第7天使用动静脉泵,每天持续12h,其余时间进行常规抬高患肢及功能锻炼,术后当天给予塞来昔布200mg口服2次/d,术后第1天使用动静脉泵镇痛,如出现中度或以上疼痛时给予临时口服奇曼丁或肌内注射杜冷丁。对照组常规抬高患肢及功能锻炼,术后口服塞来昔布200mg,2次/d,持续3～5d,期间如出现中度或以上疼痛时给予临时口服奇曼丁或肌内注射杜冷丁。观察比较两组患肢周径、肿胀程度及疼痛程度。结果入院后第3天,术后第3天、第7天观察组患肢周径明显减小,肿胀减轻率明显高于对照组,差异均具有统计学意义(P<0.05);手术当天、术后第1、第2天两组患者疼痛程度比较,差异均无统计学意义(P>0.05)。结论胫腓骨骨折患者术前术后应用动静脉泵均有较好的消肿镇痛效果。

Objective To explore the effect of AV impulse pump on pain and swelling-alleviating for patients with tibia and fibula fracture.Methods 69 patients were,according to their hospitalization serial number,divided into the observation group （n=33） and the control group （n=34）.The observation group were offered AV impulse pumping from admission to 1 day before the surgery,and from the first to seventh day after the surgery,each pumping for 12 h.During other time,the patients were asked to raise their affected limb and do functional exercising.On the day of surgery,they took 200 g of celecoxib orally,twice a day.In the event of medium or greater pain,oral administration of tramacotin or intramuscular injection of dolantine was given.The control group were only asked to raise their affected limb and do functional exercising,and after the surgery,took 200 g of celecoxib orally,twice a day,for 3 to 5 days,and oral administration of tramacotin or intramuscular injection of dolantine was given in the event of medium or greater pain.An observation and comparison was made of the circumference,swelling and pain of the affected limb between the two groups.Results On the third day after admission,and on the third and seventh day after the surgery,the observation group＇s affected limb＇s circumferences were significantly smaller,and the swelling significantly less,than those of the control group,with the difference being significant （P0.05）.On the day of surgery,and on the first and second days after the surgery,no significant difference of pain exhibited between the two groups （P0.05）.Conclusion The pre-surgery application of AV impulse pump yields sound pain and swelling alleviating effect for patients with tibia and fibula fracture.
