利尿剂在高血压治疗中对代谢的影响

摘要 目的:观察利尿剂吲达帕胺在轻中度高血压治疗中对钾、糖、脂代谢的影响。方法:无代谢异常的轻中度高血压患者103例分为吲达帕胺组和尼群地平组,进行前瞻性、随机双盲对照临床试验。结果:吲达帕胺组与尼群地平对照组经6个月治疗,糖脂代谢均无显著异常变化,但吲达帕胺组血钾水平治疗前后有显著性差异(P<0.05),而尼群地平组则无异常(P>0.05)。血浆胰岛素水平在餐后60、120分钟也有升高现象(P<0.05),其余相关指标(OGTT、血脂、体重指数)均无异常发现。结论:吲达帕胺作为一种长效噻嗪类利尿剂具有良好的降压效果,长期应用要监测血钾水平,必须定期监测随访,对糖脂代谢无影响。 objective: To Compare the different effects on potassium metabolic, glucose metabolic, and plasma lipids metabolic of Hypertansion patients. Methods : 103hypertensive patients were randomly double- blind divided into indapamide group and Nitrendipine group received the freatment for six month, The OGTr, surum potassium,piasma lipids were measured before and after treatment. Result: After six month treatment indapamide and Nitrendipine Bp was significally decreased (P 〈 0. 05). clucose and lipids metabolic effective, Were not significally, but the levels of serum potassium in indapamido were lower than Nitendipine, has different between two groups(P 〈0. 05). The levels of serum Insulin on past diet 60 min and 120min have increased( P 〈 0. 05 ). others not signiffically. Conclusion : effective of Thiazide Diuretics - indapamide have use of long time could be focus the levels of serum potassium but were not effects on the glucose metabolic and lipids metabolic. Significant and stable antihypertensive effects.
