替米沙坦对原发性高血压患者血管内皮功能的影响 被引量:2

摘要 目的探讨替米沙坦对原发性高血压患者血管内皮功能的干预作用。方法对4Q例轻、中度高血压患者(治疗组),给予替米沙坦40mg/d口服,血压下降不理想者则增加至80mg,连用12周。观察治疗前后患者血压的变化,并进行血浆ET-1、NO水平的测定,并采用超声方法检测肱动脉内皮舒张功能(FMD)的变化另选30例健康体检者作为对照组。结果对照组收缩压(108.2±9.1)mmHg、舒张压(78.6±8.4)mmHg、NO(127.8±34.6)mmol/L、ET-1(61.9±12.8)ng/l、肱动脉FMD(15.9±3.7)%。治疗组治疗前收缩压(172.8±14.5)mmHg、舒张压(114.5±12.7)mmHg、NO(54.7±12.4)mmol/L、ET-1(120.4±27.3)ng/l、肱动脉FMD(7.4±2.5)%;治疗后收缩压(110.6±9.4)mmHg、舒张压(82.9±7.6)mmHg、NO(105.2±27.4)mmol/L、ET-1(72.5±18.9)ns/l、肱动脉FMD(12.6±4.9)%。患者血浆NO水平较对照组明显降低,ET-1水平明显升高,且肱动脉FMD明显降低(均P〈0.01);经替米沙坦治疗12周后,治疗组收缩压和舒张压均控制于正常,血浆NO水平明显升高,ET-1水平明显下降,且肱动脉FMD明显升高,与治疗前比较有明显统计学差异(均P〈0.01)。结论高血压患者存在血浆NO/ET-1平衡紊乱,血管内皮功能障碍。替米沙坦具有良好降压效果,能纠正血浆NO/ET-1平衡紊乱,改善原发性高血压廊管内皮功能,且不良反应少。
